Proof of Residency:

Proof of Age and Identity:
- Birth certificate
- Passport
- State-issued identification card
Immunization Records:
- Records of required vaccinations
- Medical exemption forms (if applicable)
Previous School Records:
- Transcripts from previous schools attended
- Report cards
- Letters of recommendation (if applicable)

Guardianship or Custody Documents:
- Legal custody papers (if applicable)
- Guardianship documents (if applicable)

Health Examination Form:
- Completed and signed health examination form from a medical professional
Emergency Contact Information:
- Names, addresses, and phone numbers of emergency contacts
Proof of Legal Guardianship:
- Court orders establishing guardianship (if applicable)
Special Education or 504 Plan Documents:
- Individualized Education Program (IEP) or 504 Plan (if applicable)
